Bootstrapping technology to empower the future of individualized care

Join the team behind InterScripts and Children’s Hospital & Medical Center to present how they bootstrapped a new application designed to dramatically increase patient healthcare outcomes during an emergency. Their goal is to improve outcomes by empowering pediatric patients and their families to take control of their health, make informed decisions, and quickly respond to potential health issues. By providing easy access to individualized care plans, medical guidance, resources, and communication with healthcare providers, AdaptCare can help families stay informed and take proactive steps to manage their health while saving healthcare organizations.
Children with Medical Complexity (CMC) are a vulnerable population that require special medical treatment in the hospital and home community. When emergencies occur at home, many of these children are treated by local EMS, such as fire/rescue and Emergency Departments (ED). EMS and ED have minimal training caring CMC, so they are underprepared to address this population’s special needs leading to increased harm to the patient due to delay in treatment, unnecessary tests or procedures, and serious preventable errors. Project Austin and The Patient Assistance Team for Children’s Health (PATCH) programs provide children with individualized emergency medical and adaptive care plans, respectively. These care plans are put into the hands of emergency responders as a guidance to emergency care of the child.
Concisely put, this team has applied a unique cadre of technologies to ensure CMC in Nebraska (and beyond) receive specialized care, saving their lives in emergent situations. This project called “impossible to scope” by numerous others, yet persistence and a calling to care for others among this collaborative team gave them the inspiration and motivation to make “impossible”, a reality. There was no “playbook” or framework to work from, Project Austin and PATCH, came together with InterScripts to develop an app that has become a resource for first responders, children, and parents to provide critical care.
By providing this valuable resource to first responders and ED healthcare teams, medical care to the child is improving when it matters most, during an emergency. This improvement in medical care has a direct impact on morbidity and mortality, and ultimately healthcare costs. Through the compilation of data over a five-year time span, a 55% financial cost avoidance, or approximately $221,500,000, directly correlated to a 50% decrease in overall hospital length of stay, 27% reduction in hospital admissions and 23% reduction in ED visits was yielded.
