Unlocking Patient Data: How a Clinically Driven Chart Preparation Process Improves the Continuum of Care

Description:

Every day, providers input an ever-increasing amount of patient data into the EHR. Simultaneously, the importance for healthcare leaders to understand the impact of health data increases just as rapidly. For successful EHR data migration and patient chart abstraction to occur, a deep clinical understanding about patient healthcare data is required. An untrained data analyst misses the complexities of health data. Automation begins the process of health data transfer, but it ultimately requires a clinical expert to create the most accurate version of a patient record. Inaccurate or incomplete health histories and medication records negatively impact the quality of care. To provide safe and effective patient care, clean data is key. When clean data is abstracted into the EHR with clinical oversight, it ensures that the integrity of the data is optimized resulting in better records for clinicians to use. Leveraging a clinical chart preparation program improves data quality, reporting, and patient outcomes, and leads to a reduction of labor expenditures.
